bottom this page and following the instructions.
Once you have opened the form, you can complete
the form on-screen by clicking in the appropriate check-boxes
and completing the text fields. When you have completed the
form, please print and fax the form to 978-532-0891 for review.
If you are using the free Adobe Reader, you will not be able
to save the form with your answers to your hard drive, however
you can save and print the blank form for reference. Adobe
Acrobat users can save the completed form to their hard drives,
and can send the completed form as an email attachment to
Lighthouse Manufacturing for review.
